Facebook Ads for Med Spas: How to Turn Ad Spend Into Booked Appointments

Most med spas have tried Facebook ads and most have been burned. They boosted a post, watched the likes roll in, and got nothing they could book. So they decide Facebook ads do not work for med spas. What actually happened is subtler: the ad was fine, but everything around it was missing. Facebook ads for med spas can fill a calendar, but only when the offer, the landing page, and the follow-up are built to catch what the ad sends.

Facebook and Instagram sit under the same Meta ad system, and together they are still the most direct way to reach local patients before they go searching. Targeting is precise and reach is cheap: showing an ad to a woman in your city who is interested in aesthetics costs very little. That reach is exactly why the platform punishes a weak setup. It will happily spend your budget showing a soft offer to people who were never going to book.

We run Meta ads for injector-led clinics across Canada, so most of what follows comes from watching what separates the accounts that print bookings from the ones that quietly drain a card each month.

The offer comes before the ad

The single most common reason med spa Facebook ads fail has nothing to do with Facebook. It is the offer. An ad is an amplifier. Point it at a compelling reason to book this week and it multiplies bookings. Point it at “we offer Botox and filler, call today” and it multiplies indifference.

A great ad in front of a weak offer just helps more people ignore you faster.

Before we touch a campaign, we look at what a first-time patient gets for saying yes now. A specific, time-bound reason to book beats a general “book a consultation” every time. That is not about discounting your work into the ground. It is about giving a stranger a clear, low-risk first step that fits what you want more of. Get the offer right and mediocre ads still work. Get it wrong and no amount of targeting saves you.

Why Facebook and Instagram ads suit a med spa

Aesthetics is a visual, local, high-consideration purchase people decide on by eye, which is precisely what Meta’s platforms are built to sell:

  • The targeting is local and specific. You can show ads to women in a tight radius around your clinic, in the age range you treat most, who follow the interests your best patients share. Very little budget is wasted on people who could never walk through your door.
  • The format rewards showing real work. Short video of your space, your team, and your approach does the trust-building that a static logo never will. Patients decide who to book by watching, and this is where they watch.
  • It captures demand you have not earned yet. Google Ads catch the patient already searching. Meta ads reach the one who was not searching today but books once the right clinic appears in her feed. Both matter, and Meta is usually the cheaper of the two to start.

That combination is why paid social is the core of what we build for most clinics, and why the ones who abandon it after a bad boosted post leave the most reachable patients on the table.

What breaks med spa Facebook ads

When an account is not working, the problem is almost always one of these, and none of them is the platform itself:

  • Boosting instead of running campaigns. The “Boost post” button is not the ads platform. It optimizes for cheap engagement, not booked patients. Real campaigns run in Ads Manager with a booking or lead objective, and the difference in result is not small.
  • Sending clicks to the homepage. An interested click deserves a page built for that one offer, with your credentials and a single clear next step. A homepage that lists every service scatters that attention and the booking is lost.
  • No system to catch the lead. A patient who fills in a form at 9pm and hears nothing until Thursday has already booked elsewhere. The clinics that win reply fast, every time. The ad is only as good as what happens in the hour after someone raises their hand.
  • Killing ads before they can learn. Meta needs a few days and enough conversions to find your buyers. Switching everything off after a slow first weekend throws away the data the system needs to work.

We pulled this apart in more detail in why most aesthetic clinic ads fail, but the short version is that the ad is rarely the broken part.

The rules that govern med spa ads in Canada

Meta treats health and cosmetic-procedure advertising carefully, and Health Canada plus provincial standards add their own limits. Cross them and you get ads disapproved, an account flagged, or worse. The ones that matter most:

  • No before-and-after photos inside the ad itself. Meta restricts clinical imagery in ad creative. Keep your results on your landing page and organic posts, not in the paid ad.
  • No language that targets a personal flaw. Ads that imply something is wrong with the viewer’s body get rejected. Speak to the outcome and the experience, not to insecurity.
  • No outcome guarantees. “Perfect results guaranteed” will get pulled and can invite a complaint. Qualify every claim so it stays defensible.
  • Name the treatment, not a specific drug brand. Prescription drug brands like Botox cannot be advertised to the Canadian public with claims attached, so the safer creative sells your clinic and your approach.

These limits frustrate clinics that want to run the loud ad. In practice they protect you. The account that stays inside the rules keeps running while the aggressive competitor spends a week locked out.

Run it yourself or hand it over

A disciplined owner can run Meta ads in-house, and some do it well. The honest question is whether the hours and the learning curve are the best use of your time when a chair sits empty. Managing ads properly means writing and testing creative, then reading the data without emotion through the noisy early days. We wrote about where that line falls in self-managed ads versus managed ads.

Paid ads are one part of a larger system that includes your offer, your website, your content, and your retention. If you want to see how they fit together, our complete med spa marketing guide lays the whole thing out in order, and our med spa marketing work shows how we run it for clinics across the country.

Facebook and Instagram ads are the core of what we do for injector-led clinics. If your ad spend is not turning into booked appointments, let’s talk about your clinic.

Frequently asked questions

Do Facebook ads actually work for med spas?

Yes, when the pieces around the ad are built to support it. The platform reaches local patients cheaply and precisely, which is ideal for aesthetics. Most clinics who say it does not work were boosting posts, sending clicks to a busy homepage, or leaving leads unanswered for days. Fix those and the same ads that felt like a waste start filling the calendar.

How much should a med spa spend on Facebook ads?

Enough for Meta to gather data and find your buyers, which for most clinics starts in the range of a few thousand dollars a month once the offer and landing page are ready. Spending less than that, or switching campaigns off before they learn, usually produces the “it does not work” result. The budget matters less than what the ad points at and how fast you answer the leads it brings.

Should I boost posts or use Ads Manager?

Use Ads Manager. Boosting optimizes for cheap likes and comments, not for booked patients, so it feels active while producing little. Campaigns built in Ads Manager with a lead or booking objective, a real offer, and a dedicated landing page are what turn ad spend into appointments.
